Introduction
VTE is the third most common cause of vascular mortality worldwide and comprises (DVT) and (PE),The latest figures on VTEP from NHS England showed that since a systematic approach to preventing hospital associated VTE was introduced in 2010, there has been a 15.4% reduction in deaths within 90 days after discharge
Methods
Retrospective study where 80 patients ( 40 in each cycle )had elective day case general surgical procedures at KGH on 2024 and their data collected from Bluespier(theatre database ).
Results
On first cycle patients had operation without VTE form completed and signed ( in 50 % form filled only in the front page ). We found patients indicated for VTEP and didn’t get both pharmacological and chemical prophylaxis without documented contraindication.
On second cycle Increased number of VTEP forms correctly filled
Conclusion
Improvement is needed with regards the compliance with the DH VTE risk assessment tool and VTEP prescription when indicated .

- Disseminate findings of this audit ( presentation in Departmental + emails )
- Educate staff members about the importance of documentation (presentation + emails )
- Introduce posters in staff working areas as reminders
- Reminder on Handover
- Create screensaver on trust computers .
- Request for the IT team to make the form online .
- Emails sent to the registrars involved in the sample with feedback about the compliance .
