Early Vs Late Laparoscopic Cholecystectomy post ERCP for Cholelithiasis with Choledocholithiasis.

Introduction
Cholelithiasis accompanied by Choledocholithiasis are commonly treated in two stages: ERCP followed by laparoscopic cholecystectomy (LC). However, the optimal timing of LC after ERCP remains debated. This study explores the clinical outcomes of early versus delayed LC post ERCP, aiming to clarify the benefits and drawbacks of each approach.

Aim
To compare the surgical outcomes, complication rates, and overall effectiveness of early versus late laparoscopic cholecystectomy following ERCP in patients with cholelithiasis and choledocholithiasis.

Objectives
To study and compare the outcome of early LC vs late LC post ERCP in terms of Effective dissection of Calot’s triangle
To study and compare the outcome of early LC vs late LC post ERCP in terms of Intraoperative bleeding, operating time, conversion to open surgery and complications

Materials and Methods
This is a prospective observational study which was conducted at the Department of General Surgery, KIMS, Bhubaneswar, over a period of two years (Feb 2023–Jan 2025). A total of 150 post-ERCP patients with cholelithiasis and common bile duct stones were included and divided into two groups. Patients were followed prospectively, and data were analyzed using SPSS v26. Statistical comparisons were made using t-tests and Chi-square tests, with significance set at p < 0.05

Conclusion
Early laparoscopic cholecystectomy following ERCP is a safe and effective approach that offers procedural advantages, including reduced operative time and symptom duration, without increasing intraoperative challenges or postoperative risks.
