Improving Day case surgery rates for Green Light Prostatectomy

Introduction:
Green light laser surgery has emerged as a minimally invasive alternative for the treatment of benign prostatic hyperplasia (BPH), offering advantages such as reduced bleeding, shorter recovery times, and lower rates of complications. This audit aims to evaluate day case surgery rates with green light laser technology in line with Getting it right first time (GIRFT) guidelines, focusing on hospital duration, patient outcomes and operational efficiency.

Methods:
A retrospective analysis was conducted on patients who underwent green light laser surgery for BPH at our institution from January 2023 to June 2024. Data collected included patient demographics, hospital stay duration, surgical parameters, post-operative Trial without catheter success (TWOC) and postoperative complications. Surgical outcomes were assessed based on the success rates of day case procedures, defined as discharge within 24 hours of the operation.

Results:
A total of 87 patients were included in the analysis. The majority (95%) were discharged on the same day of surgery, showing a significant increase compared to traditional surgical methods. The average prostate size was 75cc (30-200) and complications were minimal, with a 93% incidence of successful TWOC and a mean TWOC time of 5 days.

Conclusions:
Green light laser surgery significantly improves the feasibility and safety of day case procedures for BPH. The high day case surgery rate, combined with minimal complications and high patient satisfaction, indicates that this approach is effective for high volume prostate size and could be adopted more widely in urological practices, while reducing cost burdens within the national health service.
