Unveiling a Mesenteric Mystery: Cystic Lymphangioma Presentation

Abstracts
Mesenteric Cystic lymphangiomas are uncommon benign lesions that primarily occur in children, often in the head and neck area, but can also develop in the abdomen with uncommon aetiology, its presentation varies from asymptomatic to severe acute abdomen.
In this case, we present a 6-year-old girl who presented to us with a long history of recurrent colicky abdominal pain that improved in the squatting position. Upon examination, a noticeable lump in the abdomen was identified. A CT scan and abdominal ultrasonography performed to establish the diagnosis. CT scan revealed a cystic mass that occupied the paraumbilical and epigastric regions.An ultrasonography revealed huge multilocular cystic lesions occupying most of the abdomen and multiple internal echoes. The mass was excised surgically through laparotomy, its size was "11x11.5cm" and the pathology examination verified the diagnosis as mesenteric lymphangioma.

Discussion
The incidence lymphangiomas are 5% of all paediatric tumours. Lymphangiomas usually affect head and neck but sometimes it can affect the intestinal mesentery which represents 1% of all lymphangiomas . Lymphangiomas are usually asymptomatic in the form of painless abdominal mass. Several complications may occur like volvulus of the intestine or even intestinal obstruction.

Conclusion
Lymphangiomas is an uncommon benign tumour that usually affects children. It affects mainly the head and neck but it can affect the abdomen in the intestinal mesentery. Mesenteric lymphangioma has the risk of severe complications like intestinal obstruction or volvulus. The best management is complete excision. In our case we did a complete excision with intestinal resection and anastomosis.
